CUSTOMER DECIDES TO REGISTER AT MY WEBSITE FOR A FREE CHOIR TASTER SESSION

All information entered is automatically entered into a database. I am currently using trackvia so integration with that is a possibility.

THE FORM:
First Name
Last Name
Email
Email repeat
Telephone Number
CHOOSE YOUR CHOIR
WHERE DID YOU HEAR ABOUT US?
QUESTIONS/COMMENTS?

Customer chooses which choir they want to register for. I need to be able to easily update this as more choirs are added.
Customer chooses which date. The date will depend on which choir chosen and the current date. For example, if the choir takes place on a Monday and the customer registers on Sunday, they will get the next day. If they register on a Monday, they will get the following Monday. Important point here: I need to be able to override this and set a specific date not chosen by the customer (for when I open a new choir). So there will be an auto setting and a manual setting.

Comment/question box for the customer to ask questions.

Once the registration is sent, they will receive a reply along the lines of:

Dear NAME
Thank you for registering for your free taster session with Magic Voices.
We have reserved a place for you on DATE at our WHICHCHOIR choir.
The address is ADDRESS

And whatever text I choose to add to the email.

I like the idea of integration into an external database like track via but other options are possible. The main thing is that I can easily query the database for whatever information I need (and export to csv) as well as add info like membership details, and other information too. I may also want to add columns to the database in the future.
ONE OTHER FEATURE. Once I upgrade a person to a ‘member’, I would like an automatic password to be generated and emailed to them. Their access to the members area will be their email address and the random password. They should be able to change this password themselves. When downgraded to non-member, their passwords will cease functioning.
