Book-like output from database data

Hi guys. Suppose I have a database with poem collections, each collection having a title and one or two subtitles. (Actually each poem in the database is categorized according to characteristics which can form the above collections.) And now I want to create a printable book with all corresponding titles AND with page headings which will display the titles af the poems included in that page, in a similar way that a dictionary has headings with the first and the last word contained in the repsective page - except that I only need the first one (the first collection's title, if more than one collection can fit in one page). I'm attaching two screenshots to illustrate the point. A php with css solution is preffered, but I'm open to suggestions. Please note that the database content is in unicode format and therefore not easy to be displayed in pdf. The blue lines are some default headings (eg. poem 1, poem 2, etc), the black ones are the poems derived from the database and the red ones are their titles, which are also derived from the database. (Of course titles should not be confused with the first letter of each poem, which is also red.) Inside the database, each poem has some characteristics associated with it, for example from which book it has been taken, which is the author, when it is supposed to be used, etc. The user should be able to choose which poems he wants to be displayed in the document to be created, according to drop-downs for each of the above characteristics and then the selected poems should be fitted into a skeleton of fixed "containers", for the final document creation.
