Convert simpleCart to a web form

We are looking at adding a simple shopping cart solution to our website. We have found a tool called simpleCart [login to view URL] that we are thinking about using. All we need is to add some code to our current html bases site to make it work and that is the easy part that we can do ourselves. The problem is that simpleCart mainly supports connections to PayPal and Google Checkout and not a simple form based checkout. We do not need to make any credit card transactions and only need a form filled out with customers delivery address and so on and then sent to us by e-mail so that we can import it to our order system. I need your help to move the information from the simpleCart shopping cart to a html/php form that user fills out with their address and so on that can be sent to us.
