[url removed, login to view] Form no longer working on my website

I have a website [url removed, login to view] which also has a redirect to the same website from [url removed, login to view]

There is a contact form on the site [url removed, login to view] but it is not working. If I run for this URL it appears that contact has been sent but we never receive email.

If I run it from [url removed, login to view] it gives me the following error below:

The form attempting to use this script resides at [url removed, login to view], which is not allowed to access this program.

If you are attempting to configure FormMail to run with this form, you need to add the following to @referers, explained in detail in the README file.

Add '[url removed, login to view]' to your @referers array.

I need this fixed ASAP so we get contact details from both URL's either by fixing script on page or I have no problem if a new contact form type is added.

The website is updated using standard HTML (Microsoft Expression Web). We do not use Wordpress / Joomla etc....

Please provide your price, how you intend to fix (repair existing script or add new form type) and how soon you can get completed. Ideally I will select a bidder who answers these questions clearly and can do this today.
