One-on-One Video Chat (OpenTok) and Site Design

We are building a website that allows professionals to meet with their clients one-on-one over the internet using private video-chat (OpenTok). Attached to this is a PowerPoint with wireframes and our high-level requirements. The basic sequence is as follows:
1. Professional schedules appointment in the system and provides client with an appointment ID.
2. Client visits website and enters appointment ID to start appointment; Provider logs in to start appointment. The client sees a message saying to hold until the professional joins.
3. The client and professional communicate using voice and video chat.
4. The client and professional end the session.
We are seeking a developer to help us:
a) Design a logo, providing 5 different designs for us to select one and provide tweaks.
b) Design a website based on provided wireframes, coming up with at least 3 sample designs for us to review before creating the rest of the site
c) Building a solution that meets all of the requirements outlined in the PowerPoint. The attached PowerPoint is a draft. A revised version will be provided upon request or when the job is awarded. Developer should use PHP, MySQL, OpenTok.
d) help us install the solution on our servers
