PHP/CSS Changes, Theme Touchups - repost 2

Project Description

Hello everyone! I have some simple CSS/PHP work to do on my website which should be pretty quick and painless.

1) I need my gravityforms theme touched up (seen here: [url removed, login to view]) for some reason in internet explorer the "next" button goes back to the original orange instead of the blue theme, I need this fixed and for it to look consistent. b) I need the form descriptions to show as tooltips (you'll see the first field on the first page does this properly, the other fields do not). c) At some point the "default text" disappeared from the input boxes (the text that says "First name", "Last name", etc). I need this added back in. d) I need to clean up the spacing a little bit.

2) I need a simple side bar that can be added to the top right of a page. This can be a widget or just a table I plug into pages, whichever you think is more appropriate. I need this to be themed (perhaps using the blue theme?) with rounded edges, as clean as possible. It's going to be used for a "buy now" button in some cases and for testimonials in others.

3) I need to add another page type for my adwords campaign. I simply need it to be the full width theme (shown on the previous task) with no header. I basically want to slide the page up and remove the wasted space from the header. Additionally, I need to make this page look better: [url removed, login to view] This will be used for the landing page of our ads so I need it to look as clean as possible, minor touch ups/changes may be necessary.
