On the weekends I'm an aerial and satellite installer and would like to create a system online where I can log all my job details. I need a simple sign up form/login form.

This is not to far from what I want. I have a time sheet from work that shows details of a typical job. I've scanned this and you'll be able to get an idea of how everything is laid out and what type of info is required.

An idea of type of data..
Customer name
Time slot
Job photo uploads
Full job notes

This is all I need. It is a very basic requirement that can be built in a few hours. I don't need design done but the portal must be presentable as I'll be using this often.

When logged in I'd like the home page to list jobs, about 10-20 per page and have the most recent job show at the top of the list. This could be a link with the customers name and job type.

That's it. A very simple system. Basic CRUD, I would do it myself but code in ruby only and my host unfortunately supports php on a linux server.
I need this completed before the end of Monday and will need it uploaded to my hosting package after payment so I can spend the week typing all my previous jobs into the system.

Just imagine you run a team of engineers and they have a PDA where all their jobs are presented to them only with my system I'll input the jobs myself.

Over time I'll require you to make updates to the system for me so more work will be available.
