Need a simple website built for a healthcare private practice. Will provide the content. Logo is attached, please use it as a guide for color scheme. Font preference is something from the Calibri/Verdana family. Need to be able to edit the site myself in the future (modify text, swap out images, etc) preferably using adobe dreamweaver. Do not need hosting and maintenance - only the final site files which I can upload to the server and publish.

About the site:

Would like top navigation and a variety of page layouts to support different types of content. Total site will probably be about 10 pages:

Home - This page should have a space for photo/video frame and possibly pull in a video from Youtube
About Us
Chronic Pain (overview)
Mental Illness (overview)
Treatments for pain (does not need a separate link in primary navigation, can be nested)
Treatments for mental illness (does not need a separate link in primary navigation, can be nested)
Contact US page, including a form. This should be an email submission form

Would like "click to call" enabled on phone numbers throughout the site.

Need someone who understands and can implement SEO on this site as well.

Ideally, would like Facebook and Twitter integration on something like a "What's New" page which would incorporate a feed of our Facebook and Twitter related pages (not built yet). This could be a possible extension of the project at a later time.
