A patients (name noted) comes to clinic, (the day, date and time is noted), doc checks him/her for the skin disease (disease noted), then doc treats him (treatment noted), patient's buys some skin products that would last for say 30 days (any number of days) (days noted) mode of payment noted.
We need to store this information in a database. Now we also need a reminder attached to the database that would remind us if any of the patient/s skin product or medicine is going to finish, means in above case his/her product would finish in 30 days so from the database we need a reminder that is emailed to us that that persons medicine or product is going to finish so that we can call him/her to remind schedule an appointment.
So hypothetically the database would look something like this:

Reminder (code, such that the the program would send an email containing name and phone of the patients who would be called and ask to make an appointment.)
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
