on our website (Is an Association No-profit), we need to implement this page: [login to view URL]

We need to add:

1) First line: Category of Subscriber. One is Free "Socio Ordinario" and other one is with donation "Socio Sostenitore".

In the case of "Socio Sostenitore", shall appear a field where subscriber can decide how to donate with Paypal button. Pre-filled 50 Euro.
