Forms Coding with Wordpress

I have created a user management system in Wordpress where users can login securely. I want each logged in user to be able to add and edit data using 6 predefined forms. At any stage the logged in user must be able to add a form, edit a form or view their past entries in each of 6 categories. The user must only be able to edit their own form.
I am using Twitter Bootstrap for layout so knowledge of this is important.
You will be required to
- create the 6 forms for adding data
- save the data to the database
- retrieve the form data on a per user basis
- Enable the form archive for each of 6 forms so that the user can view old entries
- Make old entries editable so that at any time the user can edit and old entry for any one of the 6 forms
There are 6 forms in total and I will show you these before the project is awarded. I attach one form as an example. In some cases the number of entries that the user will add is unknown so you must use jQuery to allow the user to add another form row if required. (To understand what I mean see this question for an example of adding a row dynamically: [url removed, login to view])
This project MUST be completed by September 13th so do not apply unless you are ready to complete by that date. Failure to complete the project by that date will result in the project being marked incomplete.
Please quote Bootstrap in your reply so I know you have read this question.
