Multi-page form for wordpress.

I am looking a Hardcoded multipage form. Not a plugin. I have an existing form but I need the code improved by a skilled coder. I want the script to be more manageable by referencing a central file with all of the variable values included. Such as: The recipient/s email addresses ($email1, $email2, $email3) Item options ($item1, $item2, $item3) This is so that the single file can be updated and the form will display those changes. The existing form has some Javascript verification on a few of the options (required fields etc.) and there is currently a Javascript date selector. The script needs re-written to perform the task better. I want an individual developer who can write the code and use their expertise to recommend better solutions than those already thought of.
