I want a survey implemented on a Wordpress-based website using PHP5 and MySQL.

Here are the specs:

-- I want users to provide their name and email and password to identify. This is NOT part of the Wordpress user management. These names/emails have to be stored in an SQL table that goes into the SQL DB associated to the Wordpress site.

-- User registration is not mandatory.

-- Users can also provide a "group name" and "group password" - more in a moment - that will give additional functionality.

-- Users will answer 12 questions on a 0% -100% scale. Preferably, this looks nice. I am considering sliders for these rating scales. Each question will come with a key word and a description is to be shown in a tool tip kind of way.

-- User ratings will have to be stored along with their name and email - if they logged on.

-- All these 12 questions will be on one page - along with the authentication process. I want to keep it simple.

-- Based on a user's rating, a radar chart / spider graph has to be generated and shown on a separate page.

-- The spider graph should show individual ratings AND the average based on all ratings over all users who took this survey.

-- If someone provides a group name, the chart generated shows the average of all responses from users who provided the same group name AND the average based over all responses.

I want to keep cost low. You don't have to write this all from scratch. There are some Wordpress plugins for surveys with data storage. You should also consider amCharts or perhaps jpCharts to generate the spider graphs.
