Simple Php Form Proccessor (Flat File Database)

I want a php form processor that uses a template for the recipt/confirmation page.
and it should only recieve an email/submission submited from the list of referrers.(to advoid spam)
This form processor takes all feilds between the form tages and sends

also, should only allow 3 submits/sends per ip, per 5 hours..(spam killer)

It should get the info from hidden tages in the form...
who to send the email to, reqirted fields..etc

So user is at a form, they enter their info and hit submit the form processor takes that the info does the check and then
sends it to the outline addresses in the hidden tags.

also another feautre...i want the script on submission to send the msg to a diffrent server and input it in a
.txt/.db file (CHMOD 755/777)...and this option should be able to be disabled

so if i install this script on [url removed, login to view] ....after submited it will send the msg/submission
to [url removed, login to view]

all the script settings should be easy to edit..maybe at the top of the script or in a settings.php...

number of submits/msg allowed
list of referrers
whatever else might be added/needed
location of msg database
turn off and on the send a copy of the msg to X server
