Need some minor updates/fixes to an existing VB2008 project.

**Add the following:


"Withdrawal Reason" - text box (100 characters)

"LEP Status" - check box

"Native Language" - text box (30 characters)

"IEP" - check box

"501 plan" - check box

"ELA Assessment Post" - drop down populated by end users

"Math Assessment Post" - drop down populated by end users

"Gender" - drop down populated with "M" and "F"

"Start Date" - validated as a date in formate mm/dd/yy

"Lead Tutor/Supervisor" - drop down populated by end users


**"ELA Assessment" to "ELA Assessment Pre"

"Math Assessmnt" to "Math Assessment Pre"

**Fix the following:


**Tutoring Location issue

**"Tutoring Locations" drop down does not always populate/save correctly. It is supposed to allow the end user to add a new or select a location from the drop down. The new location will then be available when editing additional records.

The problem: Often times when a "location" is added it will populate the "ELA Assessment" and "Math Assessment" drop downs. Sometimes it also seems to change the tutoring location for other records when the current record is saved.

**ELA/Math Assessment issue


"ELA Assessment" and "Math Assessment" do not populate correctly. Should allow typing in new record, but instead is populated by the Pre Score text box. This is not the correct funcitonalilty. The Pre Score text box does not populate at all.

**Database Info/Warning:

**Access will be provided to the existing database.**

**This is connected to a LIVE database, so any testing you do should be noted so everything can be put back to how it was.

**Furture Work:


This project may lead to further development projects as we plan on taking this and creating a full-on Student Management System. This new system will need to sync to Quickbooks - so keep that in mind when bidding on the current project.
