I am a software developer in need of some design services.

I have developed a simple Web-based registration system which will be used at different venues to capture visitor info / sign-ups.

My system was developed to support multiple "templates" -a set of HTML5 / CSS / JavaScript files. The system is functional but not very attractive, as I am artistically challenged.

I need a designer to put together 3 simple screens:

1. "Splash Screen" to feature:
A. One image. It would be nice to have some CSS effects to it for shadow / blending / simple framing, etc.
B. A few text elements that basically say "Welcome to ABC Venue", "XYZ Event", "July 1st - July 3rd".
C. Two buttons - one that says "Sign Up" or "Register" (with some call-to-action emphasis) and one that says "View Registered".
2. "Registration Screen", which basically has name and address fields / text boxes and a button to "Submit" / "Register".
3. "View Registered" Screen, which will display a scrollable list or a table of registered people's names and cities (no address details).

I don't need a fully "Responsive Design" but I don't want any fixed width layouts or absolute positioning either. The screens should re-float elements nicely to make use of available screen real estate on devices with different resolutions.

If you are the approved bidder I will send you my basic "ugly" template, which has everything above, so you can apply styles and other design "magic" :)

I already have clients asking for multiple designs - different color schema, shapes, layouts, etc. to accommodate different types of events and venues. Those will eventually be implemented as additional templates, so if I like your work there will be more almost right away but I am asking for bids for a single 3 page template only.
