Banner/header for website

I have attached a jpg that explains everything I want done with this project as well as a sample of what the header should look like. Before reading any further please read the attached file. ... .... ..... read it? good. Now as it said in the jpg I'd like to bring up again that this header also has a form integrated in it. You must supply all graphics and html for this project. The website this header is going on is [login to view URL] I am re-doing the whole website's look. The graphics are going to be re-done, the color scheme re-done, and the layout is staying the same. Since I will be re-doing all the graphics on the site, a job is open for you if you do a good job with the header to complete more work on these other graphics. I prefer someone with experience in similar projects and if you have past work, I would like to see it.
