App description
Captures Accident or Incident Data with a tablet. Phone version will also be requested from the winner and extra will be paid.
PSD's Preferred, but can work with illustrator or Fireworks files. No JPEGS.

Four Sections in the app
Fast Capture
Detailed Mode
Investigation Mode
Reporting Mode

Fast Capture Mode - Can start taking photos and videos very quickly when arriving on a scene.

Detailed Mode
Type of Incident - Near Miss/Minor/Major/ Catastrphic
Provide Site Address, Weather details, People involved, Types of Injuries, Types of Equipment Involved. Record Statements from witnesses, mark up images with arrows, markers and symbols.

Investigation Mode
Identify the causes of the incident, follow up actions to be taken, list priorities of follow up actions by colour indicators, Allow users to add custom fields.
Lost time off work - hours, days, weeks, number of people and costs, type of damage to equipment

Reporting Mode
List the summary of the incident, injuries, time of work, costs etc..
PDF, CSV & Word Exporting Options

Incident Manager Screen - List view or map view with markers indicating locations.
List of incidents with at least one thumbnail, colours to indicate if follow up actions are completed or not, colours for severity of accident, thumbnails etc.

The app needs to have very good use of symbols and icons to indicate status, small flags or coloured dots etc... to help users see very quickly what is important and what isn't.

User needs options to add custom fields to modify the template to suit. Images need to be able to be associated to relevant fields.

Cloud sharing options need to be included similar to our safety audit app [url removed, login to view]

You are welcome to expand the brief a little in certain areas, as this is just the framework that we will begin with, so if you understand the brief very well, you are welcome to add some of your own touches. From there we can give you feedback on what we like.

Style/theme ideas for the mobile app
Information fields should expand and contract to minimise scrolling.

See the attached sample of an incident report form. We want to make a digital version of that, but also expand it's data capturing and reporting features.

Solid use of two main colours with one or two highlight colours. We like the colours below for the main colours.

Green/White
Green/Black or dark grey
Red/Grey
White/Blue

Google+ Events - Nice buttons and displays a lot of information in the UI - [url removed, login to view] - The Google Events feature of Google+ is somewhat relevant, as this app will later be developed to allow for adding additional people to an incident and then have all of their information stream together.

What to avoid
Avoid using cheap clip art or very detailed icons. Information needs to be very clearly laid out and solid shapes, symbols and smart use of colour for indicators is required.
