I'm simplifying our web site (been needed for a while) and I need a
single donation page. This means I'd like to have a single donation
Right now, we have several purchase / donation points. They are:

We need to have a size selector, and a picture of the shirt. We
capture their shipping address after the click.

Another pair of Paypal forms, one for one-time and one for monthly.
This one is important to my family, so I normally enclose it in the
giraffes and the Africa horizon to draw attention to it. Not required.

and on all our pages in the sidebar. You guessed it. Paypal. The user
must enter a URL and some web text. Minimum donation $10.

unlisted page, see the Paypal donate at the bottom. I would like to
have a text field for the user to send a message along with this one.,
in case they want the funds going to a specific place.

Yup. Paypal. $54/year recurring payment. Recurrence is critical. This
is our primary funding stream.
For this to be effective, we need to make sure that the consolidated
form works just like the individual forms. Suma (Informer) and the
Donor Cloud have critical callbacks, that activate a user or a link.
Without these, the process breaks. The other forms have description
data that I rely on to keep our funds straight. So we need to honor
the setup of the individual forms as much as possible.
Also, I'd like donors to sign up for our newsletter. Our default form is

form (with a checked-by-default checkbox) that would be helpful.

It might not even be possible to create a form this complex, but we
need to try. It's way too complicated for donors and we have enough
funding challenges. So I'm open to suggestions.

Oh! I love how the getafreelancer.com payment system has those cool dropdowns when you select a payment type. I think something like that might be appropriate.
