Simple REGEX Coding Task -- For Integromat/Make/ JSON files

Job Description:
I have some HL7 files that contain a body of text. The text is not formatted until I recieve it but I need to convert it intoa JSON.
What I am looking to do is have you write 5-6 REGEX to break the text down into chunks and output the values of the data that I am extrating.
Here is the format: (expected output values in brackets, not case sensitive)
1. Image Quality:
Grade: (1,2)
Comment on technical quality (if not Grade 1 comment required - include all quality issues that apply): Overexposed (dark) / Underexposed (light) / Artefacts / Improper position / Poor contrast / Poor processing / Underinflation / Mottle / Other (please specify):
2A. Any classifiable parenchymal abnormalities: (No/Yes)
2B. Small opacities:
a. Shape/Size (Indicate one primary and one secondary): (Not applicable.)
b. Zones (indicate all affected zones): (Not applicable., TBC)
c. Profusion: (0/0, 0/1, 1/0 etc...all iterations up to..3/3)
2C. Large opacities:
a. Size: (1,2, or 3)
3A. Any classifiable pleural abnormality: (No/yes)
3B. Pleural plaques: (0,1,2 or 3)
3C. Costophrenic angle obliteration: (Yes or No)
3D. Diffuse pleural thickening: (0,1,2 or 3)
4A. Any other abnormalities: (yes or no)
4B. Other abnormalities: (od plus TBC)
4C: Comments: (Mild thoracic scoliosis)
I am looking for some REGEX code that will split this text into the variables 1 - 4C. The possible values are highlighted in the brackets.
There will be some follow up work to tweak the output.
If this is something you can help with then I'd be very grateful. I have played with REGEX101 and really didn't get far but I am led to believe this is a fairly simple task.
I am using [login to view URL] similar to Zapier so that is the destination for the REGEX. If you are familiar with Make/Integromat then that would be very helpful.
