Correct 4 Customer Input Forms

Four forms on one of our customer's website are not working, but are "designed". These forms are all similar. They need to be programmed properly. Each form has about 50 fields (all similar). The input should be labelled properly to be submitted via email to a specific email address. This is a basic html website on a Linux server. Nothing fancy. Looking for someone who can complete this project promptly.
