Using our account on [url removed, login to view] setup a new form based on the attached xls sheet. The style no's, description, colour, WS$, and RRP$ fields will need to be editable by myself not the recipient of the form.
I must be able to email the form to the relevant recipients and have them input there details + State and Country and quantities under 1,2-3,4-5,6-7,8-9,10-11 which will provide a unit total, and calculate a total per style based the WS$ price. It must then create a cost, then 10% GST amount and finally the total cost. There must be a checkbox saying "I am authorised to place this order" and text box for their name to be added. They will then hit a submit button which will then send the data back to [url removed, login to view] to be collated in the PDF portfolio response file. They should also receive a copy of their order.
