Formatting and Typsetting Manuscript for eBook and Paperback on Amazon

I need someone to take the final draft of my book and turn it into a fully formatted version that works on kindle, but also as a physical book that will be sold on Amazon.
The book is roughly 32,000 words long, and includes a few pieces of formatting where I would like a chart pulled out, or a quote expanded and italicised for emphasis.
I will be ready for this process to begin at the beginning of December, but wanted to get the process started before then.
Must have experience formatting physical books.
