This is a 2nd repost of a job that I erased as a result of an admin error with the budget allocation - sorry for any inconvenience.
I am looking for an Excale VBA script (Compatible with Excel 2003 - 2010) that populates a form - as shown in the attached file.
There must be 4 text panes in the form. The first pane on the form contains a list of Tasks (The list of Tasks available is fixed). When the user clicks on the Task list, depending on what the user selects, a list of subtasks will be diplayed on the next pane from which the user can select a sub task. On selecting the sub task the third pane will be populated with the standard activities associated with that Task & Subtask combination. The contents of all three these panes are fixed and sourced from the values in the Database sheet containing the source values (see attached file for the desired structure). The fourth pane is an input text area where the user can add additional information. That information needs to be copied back to the database when the user clicks on the "Accept and Exit" button.
If the user comes back to that task at a later stage the contents of each of the panes needs to be shown - including any additional comments saved previously. In other words the form needs to be pre-populated based on the contents of the associated database sheet.
I have included a screen shot of the form (I have been working on it, but have run out of time to get it completed) so it shoud be relatively simple to create from that screenshot. Do not worry about the "Code & Colour on Sheet" input as this is not applicable

Hi, I can do it in excel 2010. Im programmer for years and very familair with VBA for excel automation. I suggest to add task id (unique) to track tasks. Best regards, Cuong NH
