Add field to responsive form and copy form style to another form

I currently have a register form for my website:

I need another field adding to my form under the 'country' field. Form needs to remain responsive.

I need this forms style copied across to another form that already contains the required fields.

The form in question is used to update user details once registered. The only field that is different is that the username field is already populated and is greyed out from editing.
