This website needs to have some spelling errors corrected and some functionality added such as The company header needs to have our company information in it, the person filling out the web form should have their name and email address added to the form. Print the form to a pdf and attach to the email form as it currently is.

Original Scope of project
Device compatibility: pc workstation, mac, android-mac smartphones and tablets
We want a web form that will allow for salespeople to input text, choose from drop-down boxes, use radio button selections, and have a comments box to request quotes from our engineering department. The web form will have between 50 and 100 fields of various types that will need to be filled in or the questions answered. After the data has been collected and is ready to submit a pop-up screen with all the data filled in should appear and give the user a chance to go back and make corrections or go ahead and submit the request. Once the submit button has been pressed the data should be put into an organized format and sent via e-mail to our engineering department also the person that is requesting the information should have an option of inputting an e-mail address.
The request should be able to be saved and recalled later for reuse (cloned) or modified and resubmitted as a revised RFQ.
This must be a complete working form/ system. It must integrate with our existing web site.
