Outline:
Chapter 1:
1. Attention: Differentiated service delivery (DSD) is a client-centred approach to patient care that simplifies and adapts HIV care and treatment services to reflect individualized care for people living with HIV. It aims to reduce unnecessary burdens on health systems and allows refocusing of resources to those in dire need using a public health approach.
2. Insert the 4 boxes with sticky figures and question mark (only) for this section: use that to describe the components of differentiated service delivery:
a. WHAT: The type of service delivered (i.e., ART refill, clinical review, or both)
b. WHERE: The location of service delivery (i.e., in a health facility [HF] or the community)
c. WHO: Patient eligibility criteria and type of service provider
d. WHEN: The frequency of services for clinical review or ART refill
Chapter 2:
1. Attention: ART naïve patients are those patients who are newly initiated on ART
2. Insert 4 boxes with
a. What: Clinical visit, Adherence, laboratory services, drug
b. Where: ART clinic
c. Who: Clinician, adherence counsellor, laboratory technician, pharmacist
d. When: Monthly
3. Attention (just a design of the “attention” for month 6 and 7, 7b)

Chapter 3:
1. Design a box with sticky figures at the right top edge of each eligibility criteria. These boxes will be parallel to each other.

a. Adults: Sticky figure of adults, adolescent (combination of males and females)
b. Children: Sticky figure of child and babysitting
c. Pregnant and breastfeeding: Sticky figure of pregnant woman and breastfeeding woman
2. Stable Patients with NO CD4 or Viral load, Insert 4 boxes of (and add the sticky figure with question mark-Male & female):
a. What: viral load request; ARV +CTX for 1 month
b. Where: ART clinic
c. Who: clinician, pharmacist, phlebotomist
d. When: routine, refill, clinic visit

3. Stable Patients awaiting CD4 or viral load, Insert the 4 boxes of (and add the sticky figure with a question mark- Male & female):
a. What: ARV +CTX for 1 month;
b. Where: ART clinic
c. Who: pharmacist/ triage focal person
d. When: routine, refill, clinic visit

4. For Fast track model, insert the 4 boxes and add the sticky figure of male, female and adolescents:
a. When: 3 monthly Refill, 6 monthly clinic visit
b. What: ARV + CTX;
c. Where: ART clinic
d. Who: clinician, Triage focal person, pharmacist

5. For Family member model, insert the 4 boxes and add the sticky figure of the family:
a. When: 3 monthly Refill, 6 monthly clinic visit
b. What: ARV + CTX;
c. Where: ART clinic
d. Who: clinician, Triage focal person, pharmacist

6. For Community ART refill group model, insert the 4 boxes and add the sticky figure of a group of people about 5 in number:
a. When: 3 monthly Refill, 6 monthly clinic visit
b. What: ARV + CTX;
c. Where: ART clinic, community
d. Who: clinician, Triage focal person, pharmacist, community group leader/ representative

7. For Community ART refill group model illustration flow:
a. Month zero:
i. mapping, enrollment and orientation ( a group of people- sticky figures with a nurse who has a paper)
b. Month 3:
i. the group will meet at the designated venue in the community ( a group of people- sticky figures with a group leader who has a paper)
ii. Group leader will meet nurse (group leader handing over paper to nurse, and by the side of the nurse is a bundle bag “labelled pre-packaged”)
c. Month 6:
i. Group meeting with clinician (sticky figure of group meeting with doctor)
d. Month 9:
i. the group will meet at the designated venue in the community ( a group of people- sticky figures with a group leader who has a paper)
ii. Group leader will meet nurse (group leader handing over paper to nurse, and by the side of the nurse is a bundle bag “labelled pre-packaged”)
e. Month 12:
i. Group meeting with clinician (sticky figure of group meeting with doctor)

8. The sticky figure of group leader collecting bundle “labelled pre-packaged” from nurse/ pharmacy.
