Simple Bootstrap Combobox

I need a simple combo box added to site code, that replaces a current text field.
The Combobox reads list in an XML file (so it is dynamic)
The specs and full source code is attached.
If you are good with bootstrap, this should be very easy.
The code may be a little bit disorganized, because I think previous coder may have done some non-standard coding...
But you should be able to easily find the code that needs to be fixed to allow combo box
