Add Cookies to a webpage and change HTML / CSS based on existence of cookie.

I have a form on [login to view URL], once user submits the form they land on a [login to view URL] page.
I would like the [login to view URL] page to set a (true/false) uniquely named cookie (with something like 30 day expiration).
Then if the same user on the same browser goes back to [login to view URL] within the 30 days the submit button needs to be hidden (type="hidden") and a small snippet of html needs to needs to be revealed in place of where the button would be, something like "you have already submitted this form."
So if user has no cookie (false or nothing) the code on [login to view URL] would be this:
<input value="Submit" onclick="emailform()" type="button" class="primary">
If user has visited the [login to view URL] and the cookie exists when user comes back to [login to view URL], this would replace (hidden/revealed, whatever) of the above code:
<input type="hidden" value="Submit" onclick="emailform()" type="button" class="primary"><h3 class="alt" style="color: red">You have already submitted this form.</h3>
Notice the type="hidden" and the extra h3.
Obviously, needs to work in all modern browsers.
I would like something simple to be used like JS-Cookie:
[login to view URL]
tutorial: [login to view URL]
And something simple like these to hide/reveal html/css:
[login to view URL]
[login to view URL]
or basically anything you know that is very simple.
You can create the standalone pages ([login to view URL], [login to view URL], and possible js files) and send those to me along with whatever libraries need uploading if any. I will implement in the actual pages with simple step by step instructions from you.
