QR Code Scanner in Web Page

I need to develop a simple php web page with QR Code scanner functionality. This page will be used to scan my customer's account number using qr code. As soon as a qr code is successfully scanned, the scanned text (account number) should be instantly posted as a string in a [login to view URL] page .
The qr code functionality should be similar to [login to view URL] and should be quick in response. The page to be optimized and must work for mobile usage. Would like to see similar work already done with QR Code scanner working in mobile phone environment.
