Simple Barcode generator

I have a barcode generator script and just need some help with completing the script.
very simple project
The user should be able to add the following
1. Username
2. Password
Barcode needs to combine both the username and password and add a value in between
For example if the username is Tom and the password is 12345 then the barcode should be
Tom\t12345
it needs to add the \t in between the username and password
It needs to allow to select of how many barcodes to print from 1 to 5 (user needs to select an option from drop-down)
Need to be able to print automatically when clicking the print button and it needs to reset and empty the inputs as soon as the print button is clicked.
it needs to print only the barcode and not the page.
the script is already almost completed just need help to finish
