ViciDial MySQL Database Connect Error

I am looking for an expert in Open Source ViciDial / Linux for MySQL database connection err-rectification. The candidate must have experience to work at code level in Linux (ViciDial) on a remote server. My client is getting stuck when mapping a lead list during the import process. He simply needs to add a few custom fields to map information into. Please have a look on attached video file (video file will be run with teamviewer 8) for better understanding of the problem. The problem is very small and related to a change in some configuration file, so the budget is set to 30$ and will be paid to the person who will do it first. The credentials will be shared to interested persons one by one and milestones will be created immediately before releasing it to the right guy. Important: I will also need similar video prepared during this process of error rectification for confirmation only. Let me know what you need to finish this job with quality result.
