The game was originally created to be by stages or level and not a big world, this is more practical for a mobile so you can see all the elements on the screen and don’t have any need for a camera that follow the fish.
Better control of the character who spawns, spawns or spawning is where the character appear on the map. Here are the actions that must be possible to make the gameplay right:
The fish must be able to eat the characters.
The fish must be able to hit and destroys the boat.
The fish must be able to perform these actions better and better, according to the upgrades the player will get.
The characters give blood to the fish when hit, panic, stop moving to emulate the death.
The boats must be able to be destroyed, first by crack then by a hole
The boats must float, then sink when destroyed by the fish.
In the menu the fish must change when he is upgraded

If we can generate a lot of stages like that we increase our revenue being able to put ad between them.
Also if we are able to put a small story in it like this one:
The piranha was taken from is native river of Venezuela to be shown in a zoo, He escaped from his jail and he want revenge on the humans. Guide him through the stages so he can become bigger and reach is native river, letting a blood river behind him.
A story is a good way to motivate people to play, they want to see a resolution, it can also guide us doing stages, like sewers or something like that.
