USER SIGN UP/LOG IN

we are looking for the sign up and log in page.
Requirements
We already have the page but having problem pls check

So we are looking for same format and some field

or you can check the Gmail sign up for reference.
we need:
1. username (after input username has to be more than 3 alpha and it has to (by Ajax) checked in database if it exists and give result with tick mark if available and cross mark if doesn’t exits
2. same applies for email
3. third is password it should check password strength either with text or line (like in Gmail sign up)
4. reconfirm password
5 forth country name I need something when the characters is typed it will suggest the country name and you can elect it if it s the one you are looking for.
6. Fifth age group it will be the 4 age group which should be selected by dropdown
7. Word verification you should add simple easy with refresh capable so that user can refresh to display different one if they can’t understand.
8. Agree with toc and pp
IT SHOULD DISPLAY THE TIPS IF USER INPUT ANY WRONG INFO OR LEFT ANY TEXT BOX EMPTY. (pls check the attachment for e.g.
sign up and log in page will be a pop up like in [login to view URL]
When user does the signup he will receive an email with confirmation code, so user have to click that code then only his account will be verified. so you have to do that part as well
plus you have to do the log in part as well, log in must also have the password reminder process followed by same process I mean user have to input the email they used while registration and they will get a link to confirm via email plus a temporary password (alphanumeric) . so user have to click the link and input the temporary password then they should be prompt with a msg asking to change new password.
