New frontend screens for existing PHP Codeigniter web app

I have a web application on the internet built using CodeIgniter (PHP) and MySQL Database. It is current a simple one screen application and I would like to get a few more screens added. The details are in the attached design document.
Some points to note -
1) All of the screens are frontend work only i.e. no database work.
2) Some of the screens requires developer to use API from available internet services
3) Very minor web design work will be need to improve the UI look and feel to include the new header
4) Google Maps, Facebook, Twitter, Flickr, Google API,Youtube API etc are utilized

*** I am not looking to pay more than 400-500 USD for this.
*** I will provide the current application code for you as the starting version of the project
*** If I like your price, I would need you to explain me in one paragraph your design/approach that you will follow to make the changes.
