Php html code help needed for contact.php

My budget is 20$ My website contact form is not working and I would like to add Validation options and Calendar options for the form to pick the date [login to view URL] I would like to receive as an Excel file when it is submitted ,I am sending as an attachment my [login to view URL] file and the file with codes. It will be helpful to use to convert excel file and you will see in my form HTML and jpg files how would I want to format for excel output.
