I need to capture data from a Adobe PDF Document.
1) Using a pre-made Adobe PDF documents that come from a 3rd party,(that contain information that my program users need) I need a pre-made javascript (that can be used in Acrobat Pro Version 7, this javascript would be placed in the javascript folder in the standard install path of Adobe Acrobat, the users will open the PDF Documents with Adobe Reader verion 7), this javascript needs to write 3 different values (multiple times), "textgroup1", "textgroup2" & "textgroup3" writing the values associated with a checkbox, based on a checkbox being checked or not, and write the values into a MS Access database.
2) I would expect to copy and paste the different values into "place holder" locations in the javascript, unless you know a better way to capture the data.
3) There will be multiple selections in the PDF document that will all have, "textgroup1", "textgroup2" & "textgroup3" associated with a checkbox being checked or not, these selections will all need to be written to the MS Access database.
4) I will provide the MS Access database.
5) I will also need a javascript to place on a button on the PDF Document to close the PDF Document and write the data to the MS Access database.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
