This was a word document, that I would like to have as a web page.
I will need the answers to be emailed as a pdf with the questions, so when printed it looks like a word doc, and I will also need a csv of the answer put in a ftp location to be processed by a different database, I manage.
The section: {Space for date of event, contract number, client name, salesperson and entertainer} I can provide many ways. I would prefer to drop this information off in a csv file to a specific folder on ftp and you job would be to import this file, then the url to the proper survey would be [login to view URL]

This is a very easy web page/site. I can provide a mysql database to store the data as well.
I will supply an ftp account for each of the data files being provided or being delivered.
To Summarize:
1. I will drop off a file into a ftp folder.
2. Your system will pick up and import this file.
3. Your system will create a web page to be filled out.
4. Once submitted, a pdf of the survey needs to email to a specified email address and a csv file will concurrently need to be stored to a specified ftp folder..
I've had plenty of experience with building dynamic pdf's and csv files and working with ftp servers from php. This project should be no problem. One thing that I'm not clear on is the format of the file that you put on the ftp server (step 1). If you want that to be a word doc, that might be a bit tricky, though totally doable.
