Feedback Site Script

Need a feedback website script.

I must work like an standard survey script with creation and administrating surveys, to generate reports for all the survey and per each of the respondent.

Instead i want the script to have user management system like this

SuperUser: Can add moderators and set how many users and how many surveys can a moderator have.

Moderator : Can add surveys and can see the detailed reports generated by they surveys. The surveys must be asigned to a specific user .

User : Used to reports. Moderator must asign surveys to each user and he is only the interface betwenn respondent and moderator. They cannot login and cannot fill any survey.

Respondent : They respond surveys by logging into site with a pregenerated password. (see Below....)

Pregenerated Passwords : Must be a numeric code that link the survey and the user to a specific survey and must be unique for each respondent.

Detailed functionality :

The super user can add moderators with detailed information like Company, Address, mail address and others and can asign a limited surveys and user that moderator can [login to view URL] moderator can create surveys that must be asign to a specific user and create password to each respondent. The respondent can participate at survey only by logging in with a pregenerated password that the moderator will send him that pass and this pass must be a numerical password that link that respondent to a specific survey. He can only answer only to that specific survey.

So the login in the completion of the survey must be only by password.
