Modify PHP scripts to create PDF

Modify existing php scripts to generate PDF.
Modify 6 existing php scripts to include additional writes to DB (add fields to MySQL DB too), and generate PDF. The scripts are all the same (just different descriptive text for course title).
A sample of current php script attached (password is pmpsuccess885 to view html certificate).
Requirements are as follows:
1) Remove password, password will no longer be required.
2) Add additional fields for user to complete as follows:

3) Instead of generating html page, create PDF, display pdf, and also email to email address inputted by user
