HR script a few more fixes 6/18/11

I need a few more fixes for my script:

1. I am getting this error when I click on Apply Leave at [url removed, login to view] : Parse error: syntax error, unexpected '<' in /home/user/public_html/leave/include/[url removed, login to view] on line 211

2. I would that login boxes on my HTML pages and other pages to function so, Employees can login from home page. Employees can login from HTML pages and as they enter their information it will submit login to. Notice the secured server URL.

3. Assign Leave is not working. I added 1 day vacation for 8/11/11 an employee and put that vacation day as Pending. It does not show on Pending Leave and it does not show at employee's Leave Summary. I did it again but, instead of pending a create a new vacation day for 11/24/11. This vacation day was taken out of the employee's account but, it does not show on the list for the employee and it does not show on the admin neither.
