554521 strip formatting from form and database

I have a php page that has MS Word formatting that wasn't properly filtered out from the upload form. The page is here: [login to view URL] I've uploaded the code for this page. There are at: [login to view URL] Click on "No" to see the form. I've uploaded the code for the forms also. I want you to alter the forms to filter out any future Word formatting. I will want to you also take out any formatting from the current database. It's in MySQL. I have much more work like this, so if things work out between us, I'll be able to send you more work. I'm looking for someone who works fast and cheap and will be around for future projects.
