OSCOMMERCE RE DESIGN PACKING SLIP PAGE ([url removed, login to view])

This job is for a redesign of the [url removed, login to view] file in oscommerce shopping cart. We will only send you the [url removed, login to view] file. Unfortunately due to security you will not be given access to our servers.
We already have the standard packing slip setup but we would basically like the layout changed in oscommerce to match the design attached ([url removed, login to view]). This is a quick mock up of how the packing slip design should look when printed.
We are printing onto A4 paper which has a small label on it which you can peal off. The box in the top left will be a label that can be pealed off. Obviously we won't need the box on the actual packing slip, this is just to show where the label will be. The main address will be the customers shipping address. The return address and our logo should always be there. [url removed, login to view] shows the layout and measurements of where the label is so you can get the layout perfect.
The boxes underneath packed are blank boxes which we will use to check off the items with a pen when the items are picked.
