Create contact form using cgi/perl

I need 2 forms created. Both forms are coded in PHP. I just found out that the UNIX server is not PHP enabled therefore the page does not display properly. I need for someone to take the forms that I have and convert it to CGI with sendmail function. I will be available to test it out on my server. I need this form to work TODAY. Project will only be accepted after I can verify that the code will work on my server, due to fact that past experiences on this board work I submitted to get done did not work with accepted providers. This should NOT be a high cost project. I am interested quality of work with good response time on this particular project. I will more than happy provide good feedback for completed work. I have attached the forms that need to be converted to cgi/perl. The webserver is running Apache on Solaris 9 for reference If you have questions, put it on PMB,
