Shipping Calcuator

I already have a simple website using a hosting template. However, I need to add a shipping rate calculator for people to get quotes fast. Here is what is required.
1. Customer will select a country ==> this is based on zones
2. Enter how many packages they will ship
3. Enter the weight and dimensions of each package
4. Get shippig quotes
Backend:
1. An excel sheet with rates will be provided to enter the quote.
2. Formula will be provided by me to match the rates base on the actual weight/dimensional weight.
3. Add fuel surcharge, it changes each month
Example:

No fancy grafic design required.
Time: 5 business days.
