We are looking for someone that can develop an interactive website with a survey and result graph. Development could be done using Drupal, Joomla, or Wordpress; including a custom module presenting the survey and output graph. We would like the ability to have individual users sign in and create a unique profile for them to (at their option) store their data so they can return and reuse the survey again and again to graph changes. E.g. take the survey every 4 months for 2yrs and chart not only individual progress at that point in time; but graph the changes over time.
This will be for the survey, the “Quick Environmental Exposure and Sensitivity Inventory,” or QEESI, which we developed, validated and published in the medical literature. It is now widely used and available in several languages.
The QEESI contains 5 scales, each with 10 items, for a total of 50 items. The first scale is the Symptom Scale. The 0-10 ratings for each symptom can be plotted out on a “target diagram.” This diagram gives practitioners a sense, a sort of gestalt, about how severe a patient’s symptoms are overall—very time-saving, and patients feel like they’ve been “heard.” The other 4 scales are: chemical intolerances, food intolerances, masking and life impact.

The QEESI questionnaire and answer sheet are posted on my website. Currently, a user goes to this site, downloads the QEESI, fills it out, and takes it to their doctor. Instead, it would be easier if a patient’s scores in each category could automatically be tabulated and plotted on the symptom star, along with a summary of their responses and the interpretation sheet.
