Minor Web Design Changes

We are looking for a web designer with php experience to carry out minor visual changes on one of our websites. Please note our required changes below. 1. Removing a Picture from the homepage of our site and in its place inserting a contact form for lead generation purposes. This contact form can be copied form the contact us page of the site in question. 2. Inserting our company logo into the homepage of the site. 3. re-arranging 4 lines of text on the home page. 4. Inserting a banner into the homepage of our site which we have ready to use. 5. Creating a duplicate "contact us now" button which we currently use on the homepage of the site. If you can carry out these changes at a reasonable rate we will consider using your services for a high volume of lead generation sites which we have ready to build. When applying for this position please include examples of your previous workand note that positive feedback will be required. Payment to be made in escarow and will be made once job is completed.
