need 2 page script to store website PHP/MYSQL.

I need a script to let users enter their website URL. Users don't need to register. Admin must approve before website is displayed on index page. PAGE 1) index page, just shows all the website in our database. with Next Page button. Shows title of page and description PAGE 2) page to let users add their website, has title field, description field, submit button. PAGE 3) admin control panel. Has setting to show many listing per page, setting for various table and page background colors, row colors. And field to add category, edit category, delete category, and add subcategory. section to let admin approve new websites submission. Also need footer, header files. This is very simple. Will pay $35. GAF escrow only, pay after project completes and works 100% according to specification.
