I require a website set up that accomplishes a number of tasks.
When the visitor first comes to the landing page, the page will have a number of boxes in which the user will complete a very short questionnaire (some items will be text fields, some will be radio boxes).
When all fields are completed, the user presses a button, and the website returns a "document" that incorporates the user's input with some standard text.
Upon pressing another button, the site will then tweet a message which incorporates the Twitter handles that a user has input (one of the questions asked on the landing page), along with a link to the "document" that was output.
The document can be either fully text generated, or ideally, has the questionnaire text laid on top of an image that has the boilerplate text.
No illustrations will need to be developed. The page will be a plain, white, unadorned site except as described above. More detail along with wireframe will be provided to the winning bidder.
(You may also add a prominent link as the programmer of the site. This site has some potential for viral distribution.)
