I've got a site built in Expression Engine that uses a Formidable Pro form in Wordpress.
There is a link to the form on each page of the EE site, clicking it opens the form in modal window.

Client wants to know the page a user was on when they clicked the link to this form. I hope this can be done with some scripting?

The best way to do this is with an analytics package such as piwik, and then you only install it on the page the form is on and it'll tell you where people came from when they came on it.
